Status, History & Social Determinants


When seeking to document a patient’s “status” in the coding record, sometimes you’ll need to query a different term(s) in the Alpha Index in order to find the appropriate code. When do you search for status vs. presence vs. absence? The ICD-10-CM code set includes 10 categories of personal and family history codes, but when should those diagnoses be used in home health? FY2022 saw new codes added to the Social Determinants section of ICD-10-CM – when should these be included in home health claims? Walk through some common situations when these questions arise and learn how to use the Coding Center to find and assign these codes correctly. – Recorded April 25, 2022


 

Hypertension


Hypertension, common in the home health population, can result from – and obviously impact – a variety of conditions. While the ICD-10-CM Alpha Index lists “accelerated”, “benign”, “essential”, “idiopathic”, “malignant” and “systemic” as non-essential modifiers, there is more to hypertension coding than just slapping I10 on the patient’s diagnosis list. For example, there are dozens of codes to choose from depending on the etiology and other conditions your patient is dealing “with”. Cancer & Associated Conditions


When coding cancer, the proper process is key. When and how do you use the Alpha Index to find the neoplasm code? When is it then appropriate to turn to the Neoplasm Table? (2) If active, is it primary or secondary? malignant or benign? (3) How specific does our documentation allow us to be in identifying the site?
