Code Tracker With PDGM Validator

The Code Tracker is designed to store the primary and up to 24 secondary diagnoses you've identified while working through each patient record.

Click the in the Toolbox on the right side of the page to view all the codes currently on your Code Tracker. Within the Code Tracker window, you can:

  • drag and drop codes to resequence*
  • remove individual codes using the *
  • select all codes within your Code Tracker using the
  • or  all the codes off your Code Tracker to begin anew

*Be sure to clickbefore exiting the Code Tracker to retain your adjustments.


February 2020: We've added PDGM Validation to the Code Tracker! Once you've selected all your codes** for a particular patient, click PDGM_Validation_Button_27Feb20.PNG to see if:

  • The first-listed code in the Code Tracker is part of a primary diagnosis clinical group, meaning it is acceptable for payment as Primary Dx under PDGM
  • Any of the secondary diagnoses belong to a comorbidity group that garners a Low Comorbidity Adjustment under PDGM
  • Any two of the secondary diagnoses belong to comorbidity groups that together garner a High Comorbidity Interaction Adjustment under PDGM

**CMS allows one primary and "up to 24 secondary diagnoses that may be eligible for additional payment under the PDGM" – so the PDGM Validation checks only the first 25 codes listed.


The bar below the Code Tracker window header will show a check-mark if your patient's diagnoses have met the threshold or an exclamation point if it has not

Note that rearranging the codes in any way will clear the PDGM Validation results. Simply click PDGM_Validation_Button_27Feb20.PNG to re-run the Validator.

Again, be sure to click  before exiting the Code Tracker to retain your adjustments.
