Functional Impairment Calculator (Expert Access Level)

Click the PDGM_Calculator_Toolbox_Button_27Feb20.PNG box at the bottom of your Toolbox to open the Functional Impairment Calculator.

You can either manually enter the PDGM point total for the 8 OASIS items if you know it or use the calculator to walk through each of the item responses and let it calculate the total.

If you choose the former, click the radio button for "manually enter point total", then Next and enter the point total in the box on the next screen.

If you choose the latter, click the radio button for "Calculate points", then Next. You'll then be able to click the radio button for each item's response as marked on your patient's OASIS – clicking Next to move to the next item.

Note: For M1033, you only need to identify whether the patient's OASIS identifies at least four risks for hospitalization under responses 1-7.

Once the OASIS point total is determined, click the radio button for the clinical group of your patient's primary diagnosis, then Next to see whether your patient meets the low, medium or high functional impairment level threshold.
